This is the Autopsy Report & Death Certificate of Al Jughayfi Haskim Humoud Humaidi, a.k.a. Ismail Na-Ama, a detainee who died after being interrogated by Iraqi Police at Forward Operating Base (FOB), Hadithia. It is reported that Mr. Humaidi was subjected to prolonged interrogation before being returned to his cell. Mr. Humadi was later found unresponsive, breathless and without a pulse, CPR and other life saving measures were initiated, but failed to revive him. (NCIS Report (Death): 24SEP06-MEIZ-0058-7HMA).
